First decide under which of the Act you are to grant leave & then go through the applicable Rules i.e. central or state of that Act to find out format of maintaining the leave register.

Although the contract Labour ( R&A) Act does not provide any provision in respect of Leave.
Now in absence of this it depends on the establishment in which you have depolyed your workforce. Because all those law are applicable to Principle Employer will also be applicable to contractual workmen.
if PE is covered by Factories Act then need to give leave and maintianed leave recored as per factories Act.
if PE is covered by Shop Act then, leave need to give as per concerned shop act of that state and need to maintained record as per.

Dear Friends,
Contract Labour Act does not prescribed leave register as rightly said by our learned friend R N Khola.
Leave is granted to contract labour as per the primary law / main law applicable to him. If he is working in a factory registered under FA then as per FA. If he is working in commercial establishment then as per S&E Act. Primary law / main law means either FA or Mine Act or Dock Act or S&E Act.
Leave register is to be maintained as per the applicable primary law / main law.
