I am attaching sample forms for self-appraisal. I have had good results when I used them in my organization. Maybe the following forms will be useful to all. Normally, Form 8 is filled out by the superior, and Form 7 by the appraisee. Subsequently, discussions/performance reviews can be arranged. Some changes may need to be made based on your organizational needs. Please let me know if you also find them relevant.

The forms are quite good, but one has to combine them in a suitable fashion as a set for an overall review with a summary of findings. Can you do this? I will also try and inform you when complete. It was really a good lead towards something useful. Congratulations.

aNkur i don’t think you are right, the form can be used for all levels of executives- except perhaps the CEO. Uday

I am unable to download the attachments. Are the attachments in Word XP or some other version? If it's Word 2007, many may not be able to open it if they are using an older version of Word.

Can anybody suggest how to write an appraisal letter for management seeking promotion? I am working in the logistics department. This year, I had a moderate appraisal despite my best performance throughout the year. Management is aware of this but top management has not approved the promotion. I kindly request for a draft for a self-appraisal for logistics.

Therefore, some modifications are required in the appraisal process. The feedback should be objective, for example, a tick mark type. Can anybody please share model templates? This would be very useful for all types of organizations/industries.
