EPF Private Trust - resigned from the company after serving the notice period, what is the procedure to withdraw the PF?

My previous company's PF is in the Exempted category (EPF Private Trust). I have resigned from the company after serving the notice period. What is the procedure to withdraw the PF, and how do I proceed if the company denies my PF?
