Sharayu,
You may take the feedback on the following counts:
a) Ability to listen and respond constructively to team members' ideas.
b) Ability to be open with team members about your concerns.
c) Ability to expresses disagreement constructively and dispassionately (e.g., by emphasizing points of agreement, suggesting alternatives that may be acceptable to the group).
d) Ability to give honest and constructive feedback to the team members.
e) Ability to organise resources equitably when team members need it.
f) Ability to provide assistance, information, or other support to the team members, to build or maintain relationships with them.
Additional comments: - The language used in the above text is little tough. You may simplify it further. Secondly, manager might be disciplinarian. Nothing wrong to be so but those who are task-masters are always disliked. Therefore, please note that you get accurate feedback. Feedback session should not be become occasion to hit back at the manager!

Dear colleague,
Before one attempts framing any questions, I think it is necessary to know what is the purpose of this exercise and also end goal. Is it part of Annual Performance Appraisal or a stand alone activity ?
If purpose is not clarified to the employee giving feedback about the Manager, as rightly pointed out by Mr Divekar, it is likely to degenerate into hitting exercise defeating the real purpose

Dear Madam,
If this is the purpose, focus of questions should be on whether the Manager is a good leader or not ie is he leading by example and leading from the front. Does he encourage team , appreciate their good acts, enable them to learn new things, and give enough freedom in day today working and above all give the credit to the team for performance rather than to self.
