Let me understand whether you want to do the task for your company where you are working or to act on behalf of a company as a consultant. The requirements for both are different and have legal implications.

If you are doing it for your company as a staff member, you need a 'Letter of Authorization' from the employer if it is apart from your regular tasks.

In the second case, ask the agency to issue a LOI or Work Order, which will outline all the terms and conditions (job description, do's, don'ts, duration, terms of payment, and many more things, including a termination clause) for the task you are going to perform on their behalf. It is advisable to consult with a lawyer before accepting the task. Generally, a LOI or Work Order is issued after a joint discussion or meeting on the subject matter. During such meetings, always be accompanied by someone who is knowledgeable about the subject matter or make necessary preparations beforehand to avoid future problems, especially when something is new to you.

Instead of calling it a letter of authorization, in common parlance, it is referred to as a work order or agreement. Some hiring consultants may have a format of enrollment available, so you may want to check with them. Alternatively, you can create one on your own with all the necessary terms.

It should essentially be an agreement format with all terms and conditions clearly mentioned, including the validity period, and should be duly executable as advised by our seniors here. The contents may vary depending on the type of industry you are in (manufacturing, software, BPO, etc).
