Yes, you can claim the benefit of ESI. As you were relieved in February 2017, your contribution cycle for ESI was until March 2017, and cash benefits can be claimed until December 31, 2017.

You need to visit the nearest ESI office with your E-Pehchan card, Aadhaar card, and all medical documents (Original) for the reimbursement. You have to fill in the required forms. Form 15-ESI FORM is the form for dependents' benefits claim. Please note, you will not be eligible to receive the benefits of ESIC if they have already been claimed under any other enactment.

You have confirmed that you were covered under ESI. Consequently, following your departure from service in Feb-2017, you are entitled to avail only medical facilities for yourself and family members. You will not receive reimbursement for expenses incurred for your wife in a private hospital. It would have been advisable to seek treatment at an ESI hospital. However, you remain eligible for medical benefits until 31/12/2017. Please contact the respective ESI local office affiliated with your employer.
Furthermore, when posing a question, it is important to provide all relevant information. Therefore, kindly log in to the ESI IP Portal using your insurance number for clarity. You could have also reached out to your former employer.

I agree with Srinath. Please contact your employer for details on the ESI office linked to your employer. You are eligible for all the benefits until 31st December 2017. It is advisable to meet them in person as explaining things over the phone might not be as effective.
