I have been working for a pharma company in Mumbai for the past 5 years until November 30th, 2014. Despite several reminders, my employer has not issued me Form 16 for Assessment Year 2015/16. Could you please advise me on whom I should address my complaint to regarding this matter? Additionally, would you be able to provide me with the email ID and contact address of the relevant authority?

Is TDS being deducted from your salary? If so, register yourself in the IT department so that you can view your salary at https://incometaxindiaefiling.gov.in.
If TDS is being deducted and Form 16 is not being given by the employer, first take it up in writing with your management.
