No form is required to submit while enrolling in ESIC, as everything is online now. You can circulate a format to new employees to fill in the dependent details, date of birth, address, bank account, IFSC, Aadhaar number, etc., to update it in the ESIC portal.
Once an employee leaves, you have to mention the reason for zero working days while filing the monthly returns in the portal as "left" or "retirement." So the same name will not appear from the next month onwards. No paperwork is required in ESIC.

ESI Act Forms for New Employees
1. Sir, while joining employment where the ESI Act is applicable, every coverable employee is required to fill up Form-1 (Declaration Form) and Form 1-A (Family Declaration Form). For any changes in the previously submitted forms, Form-2 is required to be completed. The same should be countersigned and submitted by the principal employer to the appropriate Branch Office of ESIC on Form-3 (Return of Declaration Forms).
2. Mr. Stephen_7, in his remarks above, has correctly stated that such forms are available online. In case you encounter any difficulties, in my opinion, you should contact the appropriate Branch Office to which your unit is attached.
3. The specimen of such forms, as mentioned above, can also be seen in the ESI (General) Regulations, 1950 (as amended).
