What is the difference between Form A & B for Maternity Process. Our office is in Gurgaon, Haryana so for Haryana which form should be required to download.

1. Form A for muster roll.
2. Form B: Medical certificate stating her pregnancy from an authorized medical practitioner.
3. Form C: Death certificate (during the pregnancy).
4. Form D: In case of a delivery miscarriage.
5. Form F: Receipt of Maternity benefit.
6. Form E: Notice under Section 6 of the Maternity Benefit Act for proceeding on Maternity leave.
