Dear All,

I need guidance regarding PF Form 20 and 10 D. The case is as follows: One of our employees (workers) has been working in our firm since May 2001. He was transferred to our new firm in July 2010 and unfortunately passed away on October 7, 2012.

What should be done in this case? Please guide.

Hi Rajshree,

My English is not good, but I try to understand. First, you need to transfer PF from the old account to the new account. Then, you fill out form no. 20 and 10D.

You are required to have some documents as per the list below:
1. Original Death Certificate
2. Birth Proof of All Members
3. Bank Account in the same bank and branch (for wife and children if their age is below 25)
4. Combined Photograph of all members
5. Form 3A of the Last 3 years

If you have any problems or do not understand my view, you can call me.

Regards
