I need an Employee Satisfaction Questionnaire in Hindi for our less-educated employees (such as skilled, unskilled, or semi-skilled labor). Can anybody help me to get this questionnaire in Hindi? It's urgent.
One more thing I want to know is whether we should include columns for EMPLOYEE NAME, DESIGNATION, and DEPARTMENT to identify who filled out the questionnaire, or if we should not include them so employees do not hesitate to provide true information. Please advise me.

If you do not mind, could you please provide more information about the firm, why you are conducting the survey, and who will be collecting the information? If you are going to conduct the survey in-house, then the workers may not provide truthful answers if personal details are asked. However, if the survey is conducted by an external person and the responses are collected by them, then the workers may be more forthcoming.
Finally, one last observation: I think you need to differentiate between "qualified" and "educated." Many from my late parents' generation did not have a degree; however, they were educated. I am rather proud to say they were better educated than some of the present MBA-qualified students.
