I worked with a company in Gurgaon (Haryana) for 3 and a half months (from 5th February 2013 to 20th May 2013). They deducted my ESI every month as my salary was Rs. 14,500/-. However, neither did I get my ESI card nor was I asked to fill any form by my company.
I sent an email to my HR asking for the IP number so that I could check whether my ESI was submitted or not, but she is not providing the same. I am aware that they have not submitted my ESI. Can somebody advise me that if my HR is unable to give the IP number, can I claim my deducted ESI amount from the company or not? There is no point in leaving that amount with the company. And can I complain about the same anywhere, as this company deducts the amount but doesn't submit it to ESI.

You can directly contact the local ESIC office with your salary slip because now ESIC-eligible employees' family photographs are being taken at the ESIC office only. Your card will also be issued once it is done.
You can also obtain your ESIC card number if it has been deducted at all and can avail benefits as well.
