Could anyone please let me know how to maintain and collate the training feedback? I need to prepare a template to collate the training feedback and present it to the management team as soon as possible. It would be great if you could share a sample template.

Kindly clarify your position in the organization and the type of organization you are working for. Also, clarify whether you are training your own staff or individuals from various other organizations.
Types of Training Offered
What kinds of training are you offering, and what is the level of the participants? What is the duration of the training? Is it induction training, on-the-job training, or skill-improvement training?
The feedback form needs to be designed to suit specific needs. If it is designed in a general way to fit various situations, it may become unwieldy and difficult to collate and process.

It is important to give details that Shri B Pugazhendi has asked for. The training feedback provides input for further analysis.
Internal and External Faculty Feedback
1.0 If it is internal faculty, it reaches him through HR. With the feedback, he would perform better.
2.0 If it is external, the company can decide to continue or not with the same faculty.
3.0 HR can analyze how the same program has generated different levels of comprehension and learning amongst the participants.
Many more can be added. However, to meet your immediate need, I have enclosed a General Feedback Form. Hope it would suffice.

Hi, I am an HR professional working with an engineering service organization. We recently conducted a 2-day PMS training session with an external trainer, and now I need to prepare an analysis to share with the management team based on the feedback I received from the participants. Please provide a template or guide me on how to collate the feedback information and evaluate the effectiveness.
Thanks in advance.

From your above post, it is not clear whether you have already received feedback from the participants or not. In case you have not received it, you can devise a suitable format. It should be designed in such a way that you can get the essential feedback and also be able to easily collate and analyze it. The format should be simple enough not to strain the participants' faculties. It should not be too unwieldy that people get bored filling it out and provide pointless remarks.
Objective of the Feedback Form
Before designing the feedback form, we should be clear about the objective of the training program. The feedback is meant to assess whether the training has achieved its objectives or not. We must also decide when to collect the feedback - whether immediately after the training, after a certain time gap, or both. Additionally, we should determine from whom the feedback is to be collected - from the participants, their superiors, or both.
Considering these factors, you can draft a format and post it here for further refinement by the experts. If you have already collected feedback and only require suggestions on how to collate and analyze it, you may share a copy of the blank format here for others to provide input.
