I need help regarding an EPF-related issue. I worked in a BPO organization, and the organization deducted EPF amounts from my salary. I left the organization in September '11 due to personal reasons without notice. I requested them to provide me with my EPF details and guidance on where to submit the application form, but they did not provide any information on the EPF procedures.

You have to consult with your HR Coordinators constantly. Take followups regularly. Upon receiving of your EPF documents, form 19 & 10 C authorised by your last employer, along with form 3A & 5/10.

You have stated that you have been given an EPF No, that's fine. Fill up Form No. 19/10C and Form 5/10, and attach a xerox copy of the bank statement. Request the BPO authorized official to attest the forms. If the BPO is not signing the form, take attestation on the forms 19/10C, Form 5/10, and a xerox copy of the bank passbook from the bank manager of the bank in which you have an account, and submit it to the Provident Fund office where the account of the BPO is maintained.

Clarification on EPF Forms and Procedures
F5 and F10 are not filled out by employees but by employers. This practice was previously in place but has now been replaced with an online system. The nominees of deceased employees should submit Form 5 under the EDLI scheme, not Form 5 under the PF scheme.
