I am a bit confused about the factory license. The issue is that the factory manager has resigned, and now I have to renew the license. Which form should be used to provide the information of the change of manager? Perhaps Form No. 03. Please clear my doubt.
Thank you.

No need to become confused. Please submit Form 2 under the Factory Act and the license fee through a Treasury challan, which will be verified by the Factory Inspector/AD (I S & H) based on your manpower and sanctioned load of electricity.
However, if your factory is in Haryana, you have to upload the same on the site of the labor department.
Apply for renewal before 30th November for the next year. If you are later than 25%, it should be deposited as a late fee.

Thanks to Mr. Dinesh Divekar for correctly pointing out that the thread originated a couple of years ago, and these forms are from 2012 and earlier. I believe most of the forms need to be filled up and submitted online, for example, Form 27 Annual Return. Comments to clear up confusions by Mr. Dinesh Divekar are highly appreciated, and we thank him for the same.
