Try searching it on the net; you'll get a brief idea about how to frame the questionnaire. To analyze the data, it will depend on questionnaires, whether they are open-ended or close-ended. You can use a simple Excel sheet and add graphs, or you can use software called SPSS. It will help you apply all kinds of tests like T-test, F-Test, Z-Test (parametric, non-parametric tests). First, concentrate on your questionnaire, and after that, it will become easy to decide how to go ahead with the analytical part.
