Hi All, In 2006, I was working with ICICI on the payroll of MAFOi and left the organization after 11 months. As I was new to the job at that time, I didn't collect any of the relieving documents or forms. Now, I have my old salary slips and PF account number with me, but the MAFOI office has been closed in that location. Please suggest how I can withdraw my PF amount.

If you know your PF number, just fill out the PF withdrawal forms, i.e., Form 10C and Form 19, and submit them to the regional PF office. Also, attach one canceled cheque, bank statement, or photocopy of the bank passbook with the form.

The employer is required to seal and sign the mentioned forms.
Dear Piyush, if you have any contact information for someone at Mafoi, please call them to obtain signatures on the forms mentioned by Prajakta. Additionally, if you feel comfortable, could you let me know in which city you were working? This way, if possible, I can reach out to them and provide you with the office address.
Thank you.

Hey Bhavik, thanks a ton for your prompt response and efforts. I tried calling the number, but it is not in service now. Currently, I am not in Agra, so I cannot visit his office. I would really appreciate it if you could provide me with an email address where I can communicate with them for my PF.

Dear Piyush Ji, It appears from your post that you are quite young and are still working with another employer. May I suggest you reconsider your decision to withdraw your PF dues for 11 months of services rendered at ICICI in the year 2006? Instead, I recommend that you transfer your old PF dues to your current PF account and also obtain a scheme certificate for EPF dues. By doing so, your employer's contribution would not be taxable income, and the scheme certificate will benefit you in the future.

You can contact the PF office and explain your problem to them. They should be able to guide you in this matter. If I am not mistaken, you can actually get the PF claim form attested by the banker where you have your SB A/c if you are unable to obtain the employer's signature, etc.

If you do not have the contact information of M/s Mafoi Management, then please fill out Provident Fund withdrawal Form 10C and Form 19. Sign the forms as required and enclose your ID proof and a copy of your bank account passbook. Take these documents to any Gazetted Officer, Bank Manager, Municipal Counselor, MLA, or MP to have them attested. After that, submit your forms to the Provident Fund Organization. You will then receive your provident fund amount in your bank account.

I am working in Rajasthan. Previously, I worked in J.P. Nagar, Uttar Pradesh. The P.F. of the previous company is deposited in the PF office in Bareilly, U.P. Is it possible to get my P.F. amount without the signature of the employer?

Hi Piyush, I think you should also check with ICICI people and ask them to help you with the contact number of someone from Mafoi. I am sure they can help.

Don't worry, I have discussed this with one of my friends who is working in the PF office in Ahmedabad, Gujarat. He informed me that the PF department is transitioning to an online system, which will be effective from the end of July 2011. After the end of July 2011, you will be able to access it online. Kindly get in touch with me.
