One of the organizations I work with has about 1200 employees. The organization uses an external employee assistance provider with a range of counseling services, including psychologists, psychiatrists, and social workers.
The cost per year is $ (AUD) per employee, which at the moment is about 687 INR. Employees can have up to 4 sessions with the EAP provider each year at no extra cost.
There is not much information about the benefits to the organization because of the confidentiality of the interviews. However, there are around 20 - 35 employees who use the service each year.
Most use is reported to be related to family problems and alcohol/drug issues. A small percentage (less than 5%) is related to workplace conflict.
The EAP provider will sometimes refer employees to other agencies for additional help, but this is at the employee's expense unless the employer negotiates to support the additional service.
I'm afraid this is not a very detailed picture, but as I say, most of the information is confidential.

In case of feedback, how does the employer get to know about the feedback?

The 687 INR is on the total headcount (1200 staff). I am writing from Australia, and the name of our provider is OCAR.
The only assessment of its value is through employee surveys where staff volunteer answers about whether they use the service and if they find it valuable. These survey results suggest it is valuable.
