You will need to specify your objective of the study and the test group on whom you would be administering the questionnaire.
Considering that you have talked about Talent Retention, which is a function of a number of processes / organizational practices namely,

3. Practices -
- People development - development of functional & behavioral competence
- Policies
- Engagement and measurement - performance-related
- Compensation - straight salaries and bonuses
- Performance-related incentives - Cash & non-cash components

4. Enablers on the job
5. Freedom provided to the employee in solving problems and decision-making, Teamwork, Open communication, Shared leadership, sense of belonging, etc.
Based on the above, you could formulate straightforward questions or questions which offer 3 to 4 options which the respondent could tick. Alternatively, you could use the forced ranking method - asking the respondent to rate on a scale of 5.
I would suggest that you consider developing 2 to 3 statements under each practice to arrive at a meaningful diagnosis.
