Designing a Competency-Based Interview Form: Any Tips or Formats to Share?

I am doing my project work in an IT company in HRD. I need to design a competency-based assessment form that could be used by the interview panel during the course of the interview and after the interview for feedback to HR. I would appreciate it if anybody could give me any format or inputs in this regard.
