I hope the title is what the form is referring to. I did check with the online Form "F" under Punjab Shops and Establishment Rule No. 13. Please fill in the title/last name along with the name, for example, Rajkumar Chauhan.

The name of the manager and the name of the establishment are also listed under 2 & 3; hence, this must be appropriate. The link to the video appended hereunder also skipped this detail in question:
