I hope the title is what the form is referring to. I did check with the online Form "F" under Punjab Shops and Establishment Rule No. 13.
Please fill-in the title/last name along with the name for example Rajkumar Chauhan.
The name of the manager and name of establishment too is listed under 2 & 3 hence this must be appropriate. The link video appended hereunder also skipped this detail in question: -
