Father's Name correction in EPF account - they asking for Indemnity Bond by employer

I have applied for EPF withdraw after resignation, they have rejected, becuase father name is mismatch, when I submit joint declaration form at EPF office, they asking Indemnity Bond by employer, employer doesn't have idea, anyone has format of Indemnity bond format so I can share with my employer and apply for the claim again.

It can be resolved by submitting joint declaration form (used to correct provident fund PF member details) This is a joint form which is to be updated by employee and employer to be signed and submitted to the Regional PF Commissioner (PF Office).
