How to find Factory Inspector visit report

Dear All,

Kindly help me out to find Factory Inspector visit report for my Factory.
Report comes in mail with link every time but my Director has not received any mail form DISH.
We will require this report for Safety Audit as well as for next time factory Inspectors visit also for Documenting in Inspection book.
