Dear Ravi Indurkar, Thanks for sharing the "Interview Assessment Form." However, just a small clarification: Why ask for the details of the family members at the interview stage? What purpose does it serve?

Secondly, who fills out this form? Will it be completed by the candidate or the recruiter? If it's the latter, wouldn't it add to the data entry workload of the recruiter?

Hi, The data (up to Column C) needs to be filled out by the candidate in the personal data form before taking the technical test.

The Interview Assessment Top Sheet
The Interview Assessment Top Sheet should include assessments of attributes such as:

- Job Knowledge
- Ability to express (Communication)
- Self-Confidence
- Positive attitude/Enthusiasm
- Analytical skills
- Knowledge of Computers
- Self-Motivator
- Quick Learner
- Team Player
- Potential for growth

Ratings of 1-5 may be given to the above attributes. Additional attributes may be added or removed based on the nature of work or industry requirements.

It is quite common to inquire about the candidate's family details during the interview stage to understand their family commitments.
