Dear Saif Khan, For the review of a probationer, there is no universal form. It will vary from one designation to another and also from one level to another. You need to design it on your own.
Common Attributes for Evaluation:
a) Team spirit
b) Learning attitude
c) Speed of learning
d) Initiative
e) Adaptability (responsiveness to suggestions)
I have not included communication skills in the above list because you must have checked these during the recruitment process.
To design the review form, take help from the interview evaluation form used during the candidate's recruitment. Additionally, you may review the duties performed by the probationer. What was expected from him/her, and did he/she fulfill those expectations?
