Just send him a letter that outlines his offenses, such as incorrect attendance procedures and provable misbehavior within the team.

Then, inform him of the penalties and the actions management proposes to take if he ignores this warning.

Never tell people they are doing something wrong without explaining the penalties if they persist in their actions.

This is a simple letter and well within your capabilities as an HR person. You have no problems telling us, so tell him the same thing.
