You need to sit down and study your organization and each staff designation. Then write the questionnaire for each designation based on what happens in YOUR organization.
The performance appraisal for a person who works as a sales rep for a chemical company is totally different from, say, a teacher in your organization. Absolutely none of this is a "one size fits all proposition." It has to be specifically designed and tailored for the relevant organization. If you lack the skills and qualifications to do this, then you need to outsource it to a professional and get it done correctly.
