How to create a Performance Matrix for an Employee (NBFC sector)?

Dear Seniors, Need your support. How to create a Performance Matrix? Management asks me to start work on it individually. we are in the NBFC sector. and we have 5 departments

1. Sales
2. Verification
3. Collection
4. HR
5. Accounts

This is the first time for me to start work on it but no idea where and how to start.
