Post-probation evaluation process
Generally, post-probation evaluation is conducted by the senior authorities of the company, including the Head of Department (HOD). This evaluation aims to assess the suitability of the candidate's recruitment and uncover any shortcomings in the recruitment practices.
However, you have been instructed to conduct a self-evaluation, which seems unusual. It appears that the HR department has not provided you with the standard evaluation form. If they had provided the form, this query would not have arisen.
Components to include in your self-evaluation
Nonetheless, you may include the following in your evaluation:
a) Learnings acquired during the probation period
b) Any suggestions for process improvement given during the probation
c) Any changes in ratios during the probation period
