You can get the copy from your state Labour Department website or from the office of the labour as do not know the state
Form P :- Register of Advances paid, Deductions for damages or losses and fines),
Form Q - Attendance Register with Leave Credits
Form R - Wage Register
Form S - Register of Employees with Hours of Work
