If you are maintaining an accident register, both under the Factories Act and the ESI Act (if the workmen are covered under the Employees' State Insurance Act), you are required to send intimation to both authorities. Form 12 under the ESI Act

Form 12 under the ESI Act is an accident report intimation to be submitted by management to ESI. You can visit the ESI employer portal and enter the employee accident report in Form 12. After submitting the details, Form 12 accident will be created in the portal. Take a printout and give it to the workmen.

Form 11 under the Factories Act

Similarly, under the Factories Act, there is a Form 11 to be filled out by the management. This form must be endorsed by the ESI dispensary to confirm the submission of Form 12 and then submit the same to the Inspector of Factories in your jurisdiction.
