Your question is little confusing. After conducting Training Need Identification (TNI) or Training Needs Analysis (TNA), the training programmes are decided. Thereafter before trainer conducts the training, he/she circulates the questions amongst the participants or to the HOD. This questionnaire is called as Pre-training Questionnaire (PTQ).
Nevertheless, your question insinuates that you wanted to do TNA. Therefore, please confirm what exactly you want.
