Gratuity at Full and final - what type of form to fill in favour of employer and amount paid by cheque or cash?

I have a problem please suggest me.one employee left from my company after 7 years. So now we should pay gratuity. So in that case we sign full and final from or other any gratuity paid form please tell me what type of form to fill in favour of employer and amount paid by cheque or cash

I presume your company is covered under Payment of Gratuity Act.
