Gratuity Payment Dilemma: Which Form to Use and Should It Be Cheque or Cash?

I have a problem, please suggest to me. One employee left from my company after 7 years. So now we should pay gratuity. In that case, should we sign a full and final form or any other gratuity payment form? Please tell me what type of form to fill in favor of the employer, and should the amount be paid by cheque or cash.
