preparing final report on post training course on financial management skills

After conducting a 5 days skills training course for the schools clerks in my region the department of education request a detailed final report to be submitted with other reqiured documantation.Please assits me with the relavant example on Ms Word to follow.

At the end of the 5-day training course, did you conduct test of the trainees? If yes, then the report on the scores of the test itself form the major part of the test. Other factors that can include are why did you conduct the training, what was the selection criteria of the trainees, what was the qualification, are participants expected interact with the customers in better way because of this course, what was the cost incurred to organise this course, how will you measure the effectiveness of the training course and when will you measure and so on.
