Need to know the format of information forms that candidates fill when the visit the company for interview? Like their previous employer, contact details, references etc.

What you are asking is called as Application Blank. You may check Google to download suitable copy of applicable blank.

Rather than take someone else's form, you would be better to look at the requirements of your organisation and design a suitable form to capture the information YOU need. What I might require for my organisation is different to what you need in your organisation.
