This is Daisy from India. I would like to know the format of the information forms that candidates fill out when they visit the company for an interview. This includes details such as their previous employer, contact information, references, etc.

What you are asking is called an "Application Blank." You may check Google to download a suitable copy of the applicable blank.

Rather than take someone else's form, you would be better off looking at the requirements of your organization and designing a suitable form to capture the information you need. What I might require for my organization is different from what you need in your organization.
