How To Design A Format Of Training Assessment Form And What Will Be KRAs?

Hi All, I have to organize Monthly Training of our employees considering theoretical and practical both aspects.
Kindly advise, what will be parameters and credentials to evaluate their delivery during training.
Please provide or advise how to design a format of Assessment form and what will be KRAs.

Dear Astha,
I have been conducting the training for the last 17 years. Taking into account the problems that Training Managers face in measuring the training effectiveness, I have designed the Training Assessment Form . It is attached to this post.
Other seniors training professionals may give their inputs about the form.
