Labor Licence Renewal - Officer Asking For Form V

Dear All,
The labour licence is made on march 2006 and after so many re new the licence is not renewed after march 2013. Now the project is running but the licence is not renewed. The licnce is not renewed since last three years.
The officer said to me also provide the Form V but the principal employer said that only once form five is issued for the project. Please help me to to renew the labour licence.

if they are unwilling to issue form V you have form XXIV (monthly basis) where the original form V is mentioned, that will resolve the issue

My querry is that a public sector company Say Ä engaged a project management company 'B'for doing all activities like designing, contracting, supervision all activities on behalf 'her. Company 'A' is situated in Chhattisgarh state. Company'B" does not engaged labour herself directly but awarding a numbers of contracts for execution with man materials complete. Say a company ''C' engaged labours for execution more than 20 and wants form-V for obtaining labour licence. Now the question arise.
1. Who will issue form -V to company Ç'as principal employer-either by Á' or by'B'. and why?
