Need Form For Appraisal For Business Analyst, Data Analyst, Sales, Support And Developers

Hi,
I joined a company and need to start appraisals soon.
Its a startup with no KPIs defined so far.
Could someone help me with KPIs and format of appraisal form for the following verticals:
Business / Data analytics
Sales Team,
Support Team
Development Team

Dear Ssaavi,

You cannot have Performance Appraisal (PA) different as per the designation. PA form remains same. Policy on PA also remains same across the organisation. What differs is Key Result Areas (KRAs). These KRAs originate from Key Performance Indicators (KPIs). Therefore, first you need to define the KPIs for your business, then derive the KRAs for each department and then do the PA after three or six months. Before doing PA, you must provide each individual his/her KRA sheet at the beginning of the PA cycle. This brings complete clarity in the PA process.
