Performance Appraisal At CA Firm

Hi Everybody,
I am working as a HR & Admn. Manager with one CA Firm based at Pune. I am handling regional office HR & Admn. Total manpower strength is 20 employee. I have to make performance Assignment appraisal formats for our company.
It's CA Firm so some employees doing their article ship also in company. we are working on various different industry like for manufacturing, automotive so my director want assignment performance appraisal form format because they want analyze performance of the employee on their assignment basis which employees taken by director. Requested to please suggest the Assignment performance appraisal formats suitable to this our directors Requirement .
