Confirmation Letter Format - appoint letter date & joining date both are necessary?

I am newly join in Hr. In my office Hr dept is also new. Now 1st time we want to issue confirmation letter to the employees. Pl send me the formats of confirmation letter. I have query that in confirmation format appoint letter date & joining date both are necessary.
