New to HR: Do Confirmation Letters Need Both Appointment and Joining Dates?

I am newly joined in HR. In my office, the HR department is also new. Now, for the first time, we want to issue confirmation letters to the employees. Please send me the formats of the confirmation letter. I have a query regarding the confirmation format: Is the appointment letter date and the joining date both necessary?
