Individual and team performance assessment methods/tools/formats to measuring?

I want to get some conventional and very updated methods of individual and team performance assessment methods/tools/formats to measuring the employee's work performance in projects especially in the international non-governmental organizations from various aspects supported by the basic human resource policy and practices.

Dear Md. Al-Amin,
While designing the measures of performance, these need to be designed according to your company's requirement. We the members of this forum are outsiders and we do not your operations, systems and processes. Therefore, what is applicable to any other company, need not be applicable to your company as well.
