What documents are required for applying BOCW with Form 1(BOCW)?

What documents are required for applying BOCW with Form 1(BOCW)?
As I am applying for BOCW in Bihar state, so I don't know what documents are required with that. (Labour department also don't know what document attached with BOCW form I) So, can anyone suggest to me what document I attached?
Also, inform me about the rate of treasury challan. As I am applying for 50 workers for BOCW. And the condition for pay 1% chess.

In Gujarat State we are following listed documents for Welfare Cess Registration. The application for is attached here with. Pl visit nearby office of Factory Inspector where you can have format for your State.
1. Application Form in Triplicate
2. Copy of Work Order of GETCO
3. Copy of Registration Certificate of Establishment under The Bombay Shops & Establishments Act 1948
4. Copy of residential address proof of Director and Authorized Signatory with self attestation (Electricity/Telephone bill etc)
5. Copy of Muster Roll
6. Commencement and estimated completion date of work
7. Date of Birth & telephone no of Director and Authorized Signatory
8. Photographs of Director and Authorized Signatory-03 each
9. Photo ID proof of Director and Authorized Signatory with self attestation
