Name Change In Bank Account And Pf Accou

Hi,
I have query regarding pf. I have quit a company in March 2015. I want to withdraw my pf. As per my bank account name is Madhumitha .S but as per pf account is Madhumitha Rangarajan my father is mentioned. I have a affidavit for name change n I have ID proof election in the name Madhumitha sharath.along with form 19 andand form 10c wat I need attach.please anyone gv me declaration format for name change.

Affidavit duly signed by a notary public stating that both the names are of one person, need to be submitted to PF Office.

It is enough if the employer gives a certificate to the PF authorities along with the claim.

Dear Mr Pon
We have got several claims settled by the PF office based on our certificates even in cases of change in the name itself (like Mani and Manirathnam and Raja and Rajagopalan etc., It is the rapport and relationship with the PF dept officials which helps - ofcourse some official may insist for affidavit.

Thank you for reply,when I enquired with my HR ,they are asking for declaration letter by me to pf office stating both name is same. If anyone can send me the letter format wat needs to be mentioned it wil be helpful.

THRE IS FORM NAMED "JOINT REQUEST" FILL THE FORM WITH BOTH EMPLOYEE AND EMPLOYER ALONG WITH IF PROOF WITH CORRECT NAME AND FORM 5 ALSO.
