Missing Relieving Letter: How Should an Employee Draft an Undertaking Letter?

If the employee cannot provide the previous company's relieving letter, what is the standard format for the undertaking letter to be filled by the employee? Kindly help me solve this issue.

You could have prepared the draft on your own and uploaded it here. Anyway, the draft of the undertaking can be as below:

To, The Managing Director ______ (name and address of your company)

Declaration

Dear Sir,

1. I hereby give the following declaration while joining ______ (name of your company).
a) I had been employed at _____________ (name of the previous company) as ______ (designation) from _____ (date) to _____ (date).
b) I had a smooth exit from my previous company. Before leaving the previous company, I have taken clearance from all the departments, and I do not have any outstanding dues against me.

2. I have provided the above declaration in lieu of the employment certificate from the previous company. If the falsification of the above information is proved, then I am liable for disciplinary action.
