Navigating the Shops & Establishments Act: What Forms Do We Really Need?

I just wanted to know what all forms/registers need to be maintained by a commercial establishment covered under the Shops & Establishments Act. I know one form, Form D, which we need to maintain on a monthly basis. Kindly guide.

Establishment address, number of men/women employed, category of workers, their salaries, etc.

Secondly, I wanted to know whether all such registers need to be maintained by the establishment.

I AM NOT ABLE TO UNDERSTAND. CAN SOMEONE EXPLAIN THE COLUMNS IN THE FORM D? DO WE HAVE TO MAINTAIN EACH LINE FOR EACH EMPLOYEE? THE FORM IS A LITTLE CONFUSING. CAN SOMEONE EXPLAIN THE 1 TO 10 COLUMNS, PLEASE.
