I suggest you remove Mr. and write their names directly.

In employer e seva software have provided annexture II 's structure file. you down load this file from eseva you will get your company's data only you will have to feed remaining data and follow the give instruction.

I think in line 20(MOHDKHAN) there is a " ." simile thats why you are getting error while uploadig.

you have to make sure that there is no ". " or any other symbles and make sure there is no space before the name and after the name.
also check the instruction that is given in online in pdf form for uploading annexure ii
