Enclosed is a safety bulletin regarding improperly installed D-Ring on MILLER safety harness. Although this was identified in MILLER products, request that we double check our fall protection harnesses (regardless of manufacturer) for the same defect. Please discard and replace defective harnesses to prevent its use.

Dear Raghu,
Thanks for sharing the improper use of D-Ring.
This is a learning for all others.
In fact we treat safety harness and one of the lifting tool and tackle.
Hence we have numbered them and we test each harness every six months.
The competent Person if rejects any item, we immediately cut it at 3 to 4 points and take photographs.
Even contractors have to produce the Test Certificate by competent person every six months. for the safety harness being used by them.
