Letter To Share Appraisal Sheet

Dear seniors,
I have to send the appraisal sheet of my reportee. i need to write him a mail mentioning that the appraisal call was good etc. etc..... It was a quarterly appraisal done in the coaching style. we discussed all the points mentioned in the appraisal sheet and then gave him points.
Would you please be able to share with me the wording i should use in this mail.
