Employees Pf (second Amendment) Scheme 2012 - DOC Download

Dear friends,
I am attaching notification dated 4.5.2012 amending Employees' Providend Fund Scheme 1952. The notification provides for:
- employer to file various returns and maintaining contribution cards in electronic mode also
- employer to provide copies of relevant forms to the employees immediately after joining the service or at the time of leaving service, as the case may be
- employer to forward application for transfer of PF within 5 days of its receipt to the concerned authorities.

Thanks but I like to know that in case of transfer of PF, is Form 13 to be submitted in the commisionerate of Current Employer or Previous Employer ?
it is very urgent to me.

Thank you Mr. Agrawal for the information.
It is interesting to see that the Amendment mentions that the 'Returns shall be submitted on electronic fromat also'. Whereas the E-Sewa portal of EPFO has been announcing that 'No need to submit other returns viz Form 5/10/12A,3A and 6A'.

If you could tell me which relevant forms we have to provide to the employees immediately after joining the service
also tell me how we can prepared the contribution card.

Dear Seniors,
I am working in Pharma Company,as earlier my company was not covered in ESIC but now it is covered before 4 months .then is it possible that company can deduct ESIC form my salary which i had taken . As my CTC is 10,000/-Per Month
so plz solve my querry by phone or mail because company is duducting all amount(for 4 month) from my salary .

I Have one doubt in PF Annual Return Softcopy sheet, in my organisation one employee willing to pay Voluntry contibution Rs.500-/ per month Rs.500*12= 6000 which column will shown in PF Annual returns soft copy column.

All returns are in one file , interesting ,ECR one time activity month wise will help each member to see their monthly contribution is correct manner.

I was working with Alp consulting Pvt Ltd from 1th March 2011 to 31st dec 2011. My Employee ID is 15298 I want to know my pf balance till date.
